Cancellations

If you are unable to keep your appointment, kindly call the office at least 48 business hours in advance to cancel or reschedule so that others may use the time we had reserved for you.

 

Note that fees will apply to no shows and late cancellations. Please take a moment to review our policy here

Discharge Policy

At NGMFM we are honored to provide care for our patients and value our long term patient relationships. For that reason, we are committed to working with our patients to resolve issues that arise in the course of our care. On rare occasions, however, NGMFM, in our sole discretion, may seek to terminate a provider-patient relationship. Please take a moment to review our policy here

Scheduling

When calling to schedule an appointment our receptionist will ask you questions so we can schedule the appropriate type of visit.

 

Please note that, given insurance company constraints, problem visits need to be scheduled as separate visits from annual preventive exam visits. We dislike this as much as you do, but insurance companies tie our hands on this.

Please arrive 15 minutes before your appointment time to complete any paperwork.

If you are transferring your care to NGMFM from another practice, please complete a records release form and send it to the other practice, requesting them to send us your records.
